For your convenience, we are providing our New Patient Pack of information here on our website. Following is our "New Client Information Form" which you may fill out online and save for your records. Once you have filled out and saved the form, please email it to the CEC office. You may also print it out and mail to our office, or give the form to your veterinarian at your first visit.

If you are scheduling a pre-purchase exam, please print out the "Pre-Purchase Exam" document, sign, and return to CEC.
